The rapidly changing healthcare landscape has put employers in a dilemma figuring out how to provide leading care management capabilities that keep employees healthy while also containing costs for employers. Now more than ever, employers need new strategies or approaches to manage gaps in care and tackle the most prevalent clinical conditions including hypertension, hyperlipidemia, oncology, diabetes and musculoskeletal disorders.

In this session, you will hear from Saba Ternikar, vice president of new product and commercial business and Dave Aveni, executive director of care management of CVS Health as they share insights about how to build care management strategy that addresses the current environment to create better health outcomes and lower medical costs.
Specifically, you will learn more about:

  • The importance of taking a total population health approach
  • How to use data analytics to identify at-risk members earlier, drive behavior change and create interventions that work
  • What to consider when designing personalized clinical engagement and outreach approaches
  • Why location matters and how to create local and connected experiences
